From this page active volunteers can:

• Contact the Volunteer Services Manager at the office nearest you.

• View, print and complete the Monthly Volunteer Services Report and return to your Volunteer Services Manager.
  • Monthly Service Reports are due at the end of each month.  Record all of your volunteer hours and miles and send this form to your Volunteer Services Manager.

• View, print and complete the Volunteer Patient/Family Visit Report and return to your Volunteer Services Manager.  This report cannot be completed on-line.  Please print, complete and return by U.S. Mail.  Keep confidential.  Do not send by computer or fax.
  • Volunteer Patient/Family Visit Reports are due as soon as the visit is complete. 
  • Remember to use black ink only.
  • For assistance completing these reports, contact the Volunteer Services Manager at the office nearest you.
