From this page active volunteers can:
· Contact the Volunteer Services Manager at the office nearest you.
· View, print and complete the Volunteer Outreach Report and return to your Volunteer Services Manager.
o Monthly Service Reports are due at the end of each month. Record all of your volunteer hours and miles and send this form to your Volunteer Services Manager.
· Patient and Family Volunteers - view, print and complete the Volunteer Patient/Family Visit Report and return to your Volunteer Services Manager. This report cannot be completed on-line. Please print, complete and return by U.S. Mail. Keep confidential. Do not send by computer or fax.
o Volunteer Patient/Family Visit Reports are due as soon as the visit is complete.
o Remember to use black ink only.
o For assistance completing these reports, contact the Volunteer Services Manager at the office nearest you.
· Bereavement Support Volunteer Contact Form - please complete the form in ink, sign and return to your Volunteer Manager. This form should be completed and returned after each contact that you have with a bereaved family member. Telephone contact counts as .5 hours. All other contacts count as actual time, including travel time. If you have any concerns after making a bereavement contact, please contact the Volunteer Manager or Bereavement Specialist for your branch immediately.
