Services We Provide

Covenant's services are available 24 hours a day, seven days a week.

The Hospice Interdisciplinary Team

Covenant Hospice patients receive care from a team of caregivers that includes:

  • Physicians 
  • Nurses
  • Social workers
  • Chaplains
  • Bereavement specialists
  • Home health aides and homemakers
  • Volunteers
  • Ancillary healthcare staff
  • Professional community volunteers, such as attorneys and pharmacists

Upon admission to Covenant Hospice, the interdisciplinary team develops a specific care plan for the patient. Hospice serves as an extension of the patient's attending physician, never as a substitute. Care for patients who lack a private physician will be directed by Covenant Hospice staff physicians. Patient referrals are most frequently made by private physicians; however, anyone may refer a patient to Hospice, including family members, neighbors, friends, or the patient themselves.

Nursing and Personal Care Services

Covenant Hospice nurses are specially trained in pain management and symptom control techniques that help to alleviate the suffering often associated with a life-limiting illness. Nurses make regularly-scheduled visits, provide patient and family education, skilled patient care, and are available on a 24-hour per day on-call basis for emergencies. Additionally, teams of qualified home health aides provide a wide range of personal care services for our patients, under the direction and supervision of a hospice registered nurse.

Partners In Care - Together For Kids

The PIC - TFK program provides specialized support services to children ages 0 to 21 living with potentially life-limiting illnesses. Services can include in-home nursing and personal care, counseling, supportive therapies, and volunteer services and are available to all family members and caregivers including grandparents. Social workers help patients and families to live with dignity and the highest degree of physical, emotional, social, and spiritual comfort. Social workers provide emotional support to the patient and loved ones, assist in dealing with closure issues, help in understanding and completing advanced directives, and help in understanding and utilizing Medicare, Medicaid, or other insurance benefits. Social workers may help patients and families who need financial assistance by identifying and accessing community resources.

Bereavement Services

The feelings of loss following the death of a loved one can be overwhelming. Grief is a normal, natural response that is individual and personal. Covenant Hospice provides comfort and assistance to families and loved ones dealing with grief for at least one year following the patient's death. Through our Bereavement Services, we offer:

  • Bereavement support groups/workshops
  • Educational programs
  • Remembrance Celebrations

Children's Support Services

Children's Support Services provides trained specialists to meet the unique needs of children. These services focus on dealing with a child's feelings, including:

  • Uncertainties about death
  • Appropriate ways of expressing their feelings
  • Reinforcement of positive emotions
  • Sharing special memories

Home visits, school contacts, support groups, respite activities, phone contacts, literature, and workshops are offered for children, parents, and extended family members. Children's Support Services also hosts Camp Monarch, a children's bereavement camp, several times a year throughout our service area.

Chaplain Services

Chaplains are available to provide spiritual counseling and support to patients, family members, and loved ones upon request. They can also provide assistance to find clergy of your denomination.

Volunteer Services

Volunteers are the heart and soul of the Covenant Hospice team. Hospice volunteers receive specialized training to provide companionship and support to patients and their families, as well as a range of fund-raising and administrative support services. Hospice volunteers receive free training at various locations, according to their areas of interest.

Covenant Hospice participates in the Faith In Action program, a national interfaith volunteer program funded by the Robert Wood Johnson Foundation. This program involves faith communities working together to improve the quality of life of individuals in their congregations and communities.
