Covenant Hospice seeks to provide care for patients wherever they choose to receive care. Partners In Care™ is a program established by Covenant Hospice in 1994 that focuses on maintaining the high quality of care provided to Covenant Hospice patients living in facilities beyond the traditional home setting.

Partners In Care™ has excellent relationships with more than 200 facilities to help preserve a smooth transition for patients, no matter where they are in the healthcare system.

In 2002, Partners In Care™ implemented a Certification program for Covenant Hospice staff to ensure the high quality of care attained by knowing the intricacies of the hospice-facility relationship and the regulations presiding over that partnership.
