Job Description

A professional RN specially trained in the Hospice concept to provide for the personal, physical, emotional and social needs of the patient/family unit. Incumbent works with the weekend team to facilitate the process of Admissions to Hospice. Works to provide Hospice education to the new patient and family/significant other. An active participant in the Hospice program, including education within the healthcare community.
 
 

A.                              Make home visits for physical and emotional assessment of patient/family units.

B.                               Conduct initial physical assessments of patient in the home for the purpose of               

                                        identification of immediate problems and follow-up as necessary.

C.                               Develop a written Hospice Care Plan for each patient.

D.                               Initiate complete, accurate and up-to-date Medical Records by Monday 8:30 A.M.

E.                               Communicate a thorough initial assessment to other team members on the current status of the patient/family unit.

F.                               Completes 4-8 Admissions and /or Pre-admission visits depending on complexity, and conducts support visits in all hospitals as needed.

G.                               Assist in creating and maintaining a climate that promotes growth and harmonious

                      relationship within the Hospice staff.

H.                              Utilize new learning experiences to promote professional growth and responsible for

                       updating skills through participation in seminars, etc.

I.                                 Promote a positive image of the Hospice concept throughout the community.

J.                                Call report to designated IDT members within one (1) hour of completing Admissions

                                        so the team members can follow-up in a timely fashion.

K.                              Is responsible to obtain team information, general information, etc. from Clinical

                      Coordinator and/or Branch Office Manager on Mondays and Fridays.

L.                               Competently performs all basic nursing skills. (See attached “New Employee Nursing

                       Skills List”).

M.                             Completes Clinical Competency orientation within the 90-day probationary period.

N.                              Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services.

O.                              Perform other duties as may be assigned by Admissions Clinical Manager or Branch  Office Manager.   

P.                               Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including 
                those specifically addressing disaster preparedness and response.
    

Required Experience:

A minimum of two (2) years of clinical nursing experience. Experience must be in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility. A current RN license in the State of Florida or Alabama as applicable depending on assignment. The RN must be willing to obtain a Florida and Alabama RN licenses (Hospice will pay for the second license) as requested by the organization to occasionally assist with patient coverage in both states. Reliable transportation, valid state of Florida or Alabama driver’s license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required.  Must possess current CPR certification.
