A. Responsible for providing on-call services for personal, physical, and emotional needs of the patient/family unit.
1. Responds to calls received from patients/caregivers within fifteen minutes. Visits are to be made with first call PT/PCG verbalize distress and if PT/PCG calls a second time.
2. Uses nursing assessment skills and appropriate questions to accurately evaluate problems/needs.
3. Completes assessment of patient/family unit during visits.
4. Perform treatments/interventions in accordance with the Plan of Care and physician’s orders.
5. Provides emotional support to patient/caregiver.
6. Teaches patient/caregiver as appropriate to Plan of Care.
7. Attends patients deaths facility and home patients per the PIC guidelines and completes death paperwork.
8. Follows Standard Precautions.
9. Makes follow-up visits/phone calls to patients and caregivers when the patient has been experiencing any change or problem, and/or when the primary nurse makes recommendations.
10. Arranges for needed medications and supplies.
11. Makes visit to assess patient after a fall.
12. Makes a visit to provide support to patients hospitalized during the shift.
B. Coordinates appropriate care, communicates and collaborates with Interdisciplinary Team.
1. Notifies Clinical Manager, Assistant Regional Director or Branch Office Manager, and team members by leaving accurate and detailed messages in voice mail boxes prior to going off duty.
2. Leaves message as per policy after death attendance.
3. Accurately completes documentation of visits and phone contacts, and submits documentation in a timely manner per policy.
4. Calls physician as necessary for changes in Plan of Care.
5. Initiates continuous care when needed as per policy.
6. Follows policy if inpatient care is required.
C. Actively seeks ways to develop professionally.
1. Develops basic knowledge of Hospice Medicare/Hospice Medicaid and other reimbursement
mechanisms.
2. Develops areas of expertise in hospice nursing skills.
3. Pursues professional growth through academic and continuing educational programs.
4. Attends Hospice in-services.
D. Competently performs all basic nursing skills (see attached “New Employee Nursing Skills List”).
E. Completes Clinical Competency orientation within the 90-day probationary period.
F. Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services.

12. Attends mandatory general staff meetings, and other meetings as requested by supervisor.
I. Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services.
J. Performs other duties as may be assigned by the Clinical Manager or Branch Office Manager.
K. Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.
