A. Provides continuous care to a designated patient up to 12 hours per shift in a 24-hour period following the established Plan of Care formula by the primary or on-call RN
B. Reassesses the patient/family unit at planned intervals, contributing to the ongoing updating and maintenance of a written Hospice Care Plan.
C. Maintains complete, accurate and up-to-date medical records. Documents per Plan of Care, each shift prior to completing shift. Signature and title are required on all documentation.
D. Participates in IDTcare conferences upon request.
E. Communicates on a regular basis to primary team members or to on-call nurse the current status of assigned patient/family unit.
F. Assists in creating and maintaining an environment that promotes a sense of well being within the Hospice staff in accordance with the Hospice concept.
G. Utilizes new learning experiences to promote professional growth.
H. When RN staff relief is limited, makes patient/family visits for physical and emotional assessments under the direct supervision of the Clinical Manager and/or Interdisciplinary Teams Manager, Pensacola, Branch Office Manager.
I. Promotes a positive image of the Hospice concept throughout the community.
J. Competently performs all basic nursing skills. (See attached “New Employee Nursing Skills List”).
K. May assist in the orientation of new Home Health Aides and may also assist with annual Skills List review.
L. Completes Clinical Competency orientation within the 90-day probationary period.
M. Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services.
N. Performs other duties as may be assigned.
O. Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.
