Description
The RN is a professional nurse specially trained in the Hospice concept to provide for the personal, physical, emotional and social needs of the patient/family unit. He/she works with the Interdisciplinary Team to facilitate patient care activities; provide advice, support and assistance to the team members as needed and is responsible for ongoing patient/family evaluation and assessment. The RN is an active participant in the Hospice program including community education.
 

A.                  Competently performs all basic nursing skills. (See attached “New Employee Nursing Skills Lists”). 

B.                   Conducts initial physical assessment of patient in the home.

                C.            Makes home visits for physical and emotional assessments of patient/family units.

                D.            Develops and maintains a written Hospice Care Plan for each patient and updates twice monthly and as needed.

E.                   Maintains complete, accurate and up-to-date medical records. Documents all contacts and has documentation in office per policy.

F.                   Participates in bi-weekly patient care conferences.

                G.            Communicates on a regular basis to other team members the current status of the patient/family unit.

                H.            Collaborates with Interdisciplinary Team members to ensure the best care for the patient/family according to the Hospice concept.

                I.              Writes Plan of Care when requesting a Nursing Assistant.

                J.             Makes Supervisory visits to all patients being cared for by HHA.

                K.            Requests volunteers as needed in writing.

                L.             Contacts all volunteers assigned to Nurse's patients to:

                                -               Share information before volunteer sees patient

                                -               Update volunteer

                                -               Notify volunteer of IDT meetings

                M.           Assists in volunteer training and in-services.

                N.            Notifies Clinical Manager, Interdisciplinary Teams/Branch/Residence Manager on a daily basis of all changes in status of patient, including hospitalizations and discharges. Writes weekly update.

O.                  Assists other team members in patient/family care. 

P.                   Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services.

Q.                  Assists in creating and maintaining a climate that promotes growth and harmonious relationships within the Hospice staff.

R.                   Completes Clinical Competency Orientation within the 90-day probationary period.

                S.            Utilizes new learning experiences to promote professional growth.

                T.            Promotes a positive image of the Hospice concept throughout the community.

U.              Performs other duties as may be assigned by the Clinical Manager, Interdisciplinary 
        Teams/Branch/Residence Manager.

V.               Required to comply with all Covenant Hospice Policies, Procedures and Personnel 
       
Policies, including those specifically addressing disaster preparedness and response.

Job Requirements

Experience
A minimum of two (2) years of clinical nursing experience. Experience must be in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility. A current RN license in the State of Florida or Alabama as applicable depending on assignment. The RN must be willing to obtain a Florida and Alabama RN licenses (Hospice will pay for the second license) as requested by the organization to occasionally assist with patient coverage in both states. Reliable transportation, valid state of Florida or Alabama driver’s license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required.   Must possess current CPR certification.
Skills
This position frequently requires medium/heavy lifting. Normally such lifting will involve but not be limited to, assisting with transfer/repositioning of the patient either in the home or nursing facility. The position is also mentally demanding because of the continual need of assessing and evaluating clinical aspects of the patient's care. This position requires considerable time traveling in a motor vehicle in connection with performing essential functions of position.
