A registered nurse specially trained in the Hospice concept to provide for the personal, physical, emotional and social needs of the patient/family unit. Member of the Admissions Team. Provides Hospice education to new patients and family/significant others. An active participant in the Hospice program, including community education.
A. Make initial home visits for physical and emotional assessments of patient/family units.
B. Conduct initial physical assessment of patient in the home for the purpose of identification of immediate problems and follow-up as necessary.
C. Develop a written Hospice Care Plan for each patient.
D. Initiate accurate and up-to-date medical records that must be completed by next business day for morning report to staff.
E. Participate in Admissions Team conferences as scheduled.
F. Communicate a thorough initial assessment to appropriate team members on the current status of the patient/family unit.
G. Assist in creating and maintaining a climate that promotes growth and harmonious relationships within the Hospice staff.
H. Utilize new learning experiences to promote professional growth and responsible for updating skills through participation in seminars, workshops, etc.
I. Communicate to patient/family a thorough introduction to Hospice philosophy and services available.
J. Maintain contact with appropriate physicians as assigned by Manager or Director.
K. Competently performs all basic nursing skills. (See attached “New Employee Nursing Skills List”).
L. Complete Clinical Competency orientation within the 90-day probationary period
M. Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services.
N. Promote a positive image of the Hospice concept throughout the community.
O. Perform other duties as may be assigned by Admissions Clinical Manager or Branch Office Manager.
P. Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.
