How to Get Care

Our admissions nurses are available 24 hours a day, 7 days a week and are able to travel to any location within our service area, whether it be in homes, hospitals, nursing homes, adult living facilities, skilled nursing facilities, and other residential care settings.

Call your local Covenant Hospice office or call toll free, in Florida 800-541-3072 and in Alabama 877-244-7379.

The admissions nurse will explain our services to meet your needs.

The Physician will be contacted for the Hospice admission order.

As soon as we receive the Physician's order, a Registered Nurse will travel to the patient’s location to initiate hospice services.

Upon admission, Covenant’s caring team will develop a customized plan of care for each patient, family and significant others.

Our professional and caring team will continue to provide high quality compassionate care for the patient, family and significant others. We are only a phone call away 24 hours a day, 7 days a week.
